Emily Hazelbaker, 18, of Ankeny, passed away unexpectedly on Friday, June 14, 2013.
Services will be held at 11 a.m., Thursday, June 20, 2013, at Grace Church, 4200 E 25th Street. Visitation will be from 5 to 8 p.m., Wednesday, June 19, at Hamilton’s Funeral Home, 605 Lyon Street.
Memorial contributions may be directed to the family in loving memory of Emily.
